Responsibilities

• Assess patient condition during admission and each shift; report changes.
• Document care in the patient record promptly and accurately.
• Deliver care and respond promptly to patient requests; anticipate needs.
• Coordinate with multidisciplinary team to plan and adjust patient treatment.
• Serve as interface between providers and community support for ongoing care.
