Qualifications

• Bachelor's degree in social work required; CSW certification preferred.
• Ability to meet with patients and families; document in EMR.
• Collaborate with LSWs/LCSWs for referrals and follow-up.
• Fast-paced environment; strong attention to detail; works independently when needed.

Responsibilities

• Meet with patients and families; liaise with outside agencies for referrals.
• Document in the electronic medical record (EMR).
• Assist LSWs and LCSWs in making referrals.
• Work both independently and as part of the Social Work team.
