Job summary

Northwestern Medicine seeks a Behavioral Health Counselor/Case Manager to provide individualized support to clients in permanent supportive housing, helping them achieve housing stability and independence. You will connect clients with behavioral health, healthcare, benefits, employment, and other community resources.

Key responsibilities include creating tailored service plans, coordinating with providers, assisting with social services, and documenting client progress.

Qualifications

• Bachelor’s in Behavioral Health required.
• Valid State of Illinois driver’s license and current auto liability insurance.
• Advanced verbal and written communication skills.

Responsibilities

• Develop individualized service plans and support client goals.
• Connect clients with behavioral health, medical, and community resources.
• Assist with benefits, transportation, employment, and other social-service needs.
• Support housing stability, including lease compliance and landlord communication.
• Provide crisis support and resource navigation.
• Coordinate care with healthcare providers, housing programs, and community agencies.
• Maintain accurate documentation of services, referrals, and client progress.
