Job summary

TrueCare is seeking a Behavioral Health Consultant to join its primary care team in California. You will triage and manage patients with medical and behavioral health needs, providing psychoeducation and tailored behavioral treatment plans.

The role supports providers in recognizing mental health issues and coordinating care across disciplines. Ideal candidates hold an active CA license (LCSW/LMFT/PhD), have trauma-informed experience, and can communicate in English and Spanish.

Qualifications

• Master's degree or higher in a relevant field.
• Licensed clinical social worker, LMFT, or psychologist in CA.
• Experience with health psychology and family/couple counseling.

Responsibilities

• Triages and manages primary care patients with behavioral health needs.
• Develops behavioral therapy plans and psychoeducation strategies.
• Assists primary care providers in recognizing mental disorders.
