Responsibilities

• Promotes facility mission, vision and values in services and practices.
• Conducts full client inventory in Intake area and reports to nurse.
• Notifies RN of changes and-takes vital signs per Nursing Assessment Form.
• Provides general nursing care including ADLs and transfers.
• Observes patients for changes and de-escalates when needed.
• Participates in patient care conferences and collaborates with staff.
