Job summary

Horizons in Bridgeport, CT seeks a qualified Behavioral Health Counselor to counsel individuals and groups with substance use and co-occurring disorders. Under supervision, you will develop and implement therapeutic treatment plans and monitor progress.

Shifts are Monday through Friday, 8:00 am to 4:00 pm. Responsibilities include assessments, level-of-care recommendations, crisis intervention when needed, documentation in EHR, HIPAA compliance, and collaboration with medical and case management

Qualifications

Ability to relate to people of diverse backgrounds.
Strong organizational and interpersonal skills.
Experience documenting in an EHR.
Experience facilitating groups.
Experience in co-occurring treatment.
Computer literate including MS Office.

Responsibilities

Monitor, evaluate and record patient progress against treatment plans.
Perform integrated behavioral health assessments using DSM-5 and ASAM dimensions.
Work with supervision to determine level of care.
Develop individualized treatment plans and reviews in accordance with agency standards.
Provide crisis intervention, as necessary.
Collect Urine Drug Screens (UDS) as needed.
Facilitate groups and document observations.
Coordinate with medical staff to amend plans as directed.
Maintain client records and HIPAA-compliant reporting.
Attend staff and clinical meetings; inform supervisors of critical issues.
