View Point Health in Georgia seeks a licensed social worker to provide therapeutic services for individuals with mental health or addictive disorders. The role involves assessments, service planning, court-related duties, and coordination with community resources.

The position emphasizes case management, crisis intervention, and collaboration with Drug, DUI & FTC Court teams. Requires Masters in social work and eligibility for licensure.

Formación

• Master's degree in social work and counseling from an accredited college or university and eligibility to be licensed.
• Experience with case management is preferred.]
• Prefer licensure in GA as LMSW, LAPC, LCSW, LPC or LMFT.

Responsabilidades

• Provides professional therapeutic services to persons with mental illness and/or addictive disease.
• Arranges appointment schedule at optimal intervals to maximize time.
• Conducts face-to-face assessments to identify problems and service needs.
• Writes individual service plans with required standards and timely timeframes.
• Provides group, individual, or family sessions as indicated in the service plan.
• Serves as case manager to ensure communication and continuity of care.
• Develops and maintains linkages with community providers and services.
• Attends court and staffings for Drug, DUI & FTC Court as needed.
• Completes GPRA assessments and enters data in SPARS system.
