Mental Health Cooperative in Columbia, TN seeks a Behavioral Health Clinician to provide therapy for children, youth and adults and to perform enrollment assessments as part of a collaborative care team.

You will engage individuals and groups using evidence-based modalities, develop treatment plans, coordinate with families and providers, and help connect consumers to community resources while meeting regulatory and organizational standards.

Qualifications

• Master’s degree in mental health field required.
• Licensed or license-eligible (LPC/MHSP, LCSW or LMFT) in Tennessee required.
• Ability to work with individuals and groups dealing with serious mental illness.

Responsibilities

• Engage in therapy with consumers experiencing serious and persistent mental illness.
• Utilize evidence-based therapeutic modalities to counsel consumers and supports.
• Develop and implement treatment plans collaboratively with the consumer.
• Meet with families, clinic providers, care managers and other team members.
• Provide enrollment process for referred individuals and complete enrollment documentation.

Will be responsible for conducting enrollment assessments as needed.Job Title : Behavioral Health ClinicianJob Summary : To perform a combination of behavioral health clinical tasks including and not limited to intake assessment on referrals for mental health services and/or provide counsel with emphasis on prevention and recovery as assigned on weekly rotation schedule by Program Manager. Work with individuals and groups to promote optimum mental health.This position will help individuals deal with complex and comorbid diagnosis which may include: serious persistent mental health illness, addictions and substance abuse; family, parenting, marital problems and challenges with social determinants of health; suicide; stress management; problems with self-esteem; and issues associated with aging, physical, mental and emotional health.Schedule: Monday-Friday, 8:00 AM - 4:30 PMLocation : 100 Berrywood Dr. Columbia, TNPos i tion R e s ponsibil i t i e s :Engage in therapy with Consumers experiencing serious and persistent mental illness, serious emotional disturbances, substance abuse, suicidality, relationship issues, and other stressors.Utilize evidence based therapeutic modalities to counsel Consumers and their supports, via individual, parent/caregiver, and group sessions, to assist in overcoming challenges, utilizing coping skills, and improving outcomes.Develop and implement treatment plans collaboratively with Consumer based on clinical experience and knowledge.Evaluate the effectiveness of interventions and Consumers’ progress in resolving identified problems and moving towards defined objectives.Modify treatment activities and approaches as needed to comply with changes in consumer’s status.Refer Consumers or family members to community resources or to specialists as necessary.Meet with families, clinic providers, care managers and other team members to collaborate on Consumer goals and improve their outcomes.Collaborate with other staff members, primary care practice partners including pediatricians, doctors, nurse practitioners, office staff, and referral sources as assigned to perform clinical assessments and develop treatment plans.Provide quick access to care via high responsiveness and proactive engagement to eligible individuals and referral partners.Perform enrollment process for individuals who have been referred to MHC, evaluating needs for all service lines (Care Management, Medication Management, Therapy).Complete all enrollment assessment documentation to organizational standards.Disposition Consumer to appropriate external resources for other mental health services when ineligible for MHC services.Identify individuals that are in need of crisis services rather than routine intake. If appropriate, facilitate crisis intervention.Assist in resolving any grievance received from a consumer regarding referral.Utilize Intake Committee for all consumers who meet criteria who have a history of violence, sexual crimes or serious criminal activity. Write completed intake and email to Intake Committee for review.
