Job summary

Mental Health Cooperative in Columbia, TN, seeks a qualified behavioral health professional to perform intake assessments and counseling with emphasis on prevention and recovery. The role includes collaboration with a care team to address complex and comorbid conditions and to promote optimum mental health.

This full-time position offers a Monday–Friday schedule, competitive benefits, and opportunities to work with individuals and families across several service lines, including care management

Qualifications

  • Master’s degree in Mental Health field required.
  • LPC/MHSP, LCSW or LMFT license in TN or eligibility to obtain one.
  • Valid TN driver's license and insurability.

Responsibilities

  • Engage in therapy with Consumers experiencing serious mental illness and related issues.
  • Counsel through individual, group, and family sessions using evidence-based modalities.
  • Develop and implement collaborative treatment plans with Consumers.
  • Evaluate progress and adjust interventions to achieve defined outcomes.
  • Refer to community resources or specialists as needed.
  • Collaborate with care team and primary care partners to plan care.
  • Ensure enrollment assessments and documentation meet organizational standards.
  • Provide rapid access to care and proactive engagement for eligible individuals.

Education

Master’s degree in Mental Health
Licensed or license-eligible with LPC/MHSP, LCSW or LMFT in TN

Job Summary: To perform a combination of behavioral health clinical tasks including and not limited to intake assessment on referrals for mental health services and/or provide counsel with emphasis on prevention and recovery as assigned on weekly rotation schedule by Program Manager. Work with individuals and groups to promote optimum mental health.This position will help individuals deal with complex and comorbid diagnosis which may include: serious persistent mental health illness, addictions and substance abuse; family, parenting, marital problems and challenges with social determinants of health; suicide; stress management; problems with self-esteem; and issues associated with aging, physical, mental and emotional health.

Schedule: Monday-Friday, 8:00 AM – 4:30 PM

Location: 100 Berrywood Dr. Columbia, TN

Job Responsibilities

Position Responsibilities:

  • Engage in therapy with Consumers experiencing serious and persistent mental illness, serious emotional disturbances, substance abuse, suicidality, relationship issues, and other stressors.
  • Utilize evidence based therapeutic modalities to counsel Consumers and their supports, via individual, parent/caregiver, and group sessions, to assist in overcoming challenges, utilizing coping skills, and improving outcomes.
  • Develop and implement treatment plans collaboratively with Consumer based on clinical experience and knowledge.
  • Evaluate the effectiveness of interventions and Consumers’ progress in resolving identified problems and moving towards defined objectives.
  • Modify treatment activities and approaches as needed to comply with changes in consumer’s status.
  • Refer Consumers or family members to community resources or to specialists as necessary.
  • Meet with families, clinic providers, care managers and other team members to collaborate on Consumer goals and improve their outcomes.
  • Collaborate with other staff members, primary care practice partners including pediatricians, doctors, nurse practitioners, office staff, and referral sources as assigned to perform clinical assessments and develop treatment plans.
  • Provide quick access to care via high responsiveness and proactive engagement to eligible individuals and referral partners.
  • Perform enrollment process for individuals who have been referred to MHC, evaluating needs for all service lines (Care Management, Medication Management, Therapy).
  • Complete all enrollment assessment documentation to organizational standards.
  • Disposition Consumer to appropriate external resources for other mental health services when ineligible for MHC services.
  • Identify individuals that are in need of crisis services rather than routine intake.If appropriate, facilitate crisis intervention.
  • Assist in resolving any grievance received from a consumer regarding referral.
  • Utilize Intake Committee for all consumers who meet criteria who have a history of violence, sexual crimes or serious criminal activity.Write completed intake and email to Intake Committee for review. Contact consumer with decision.
  • Learn about new developments in their field by reading professional literature, attending courses and seminars, establishing and maintaining contact with other social service agencies.
  • Meet productivity standards as indicated by department goals.
Qualifications

Essential Skills and Experience:

  • Master’s degree (M.A., M.S.) in Mental Health field
  • Licensed or License-eligible with either with LPC/MHSP, LCSW or LMFT in the state of TN.

Essential Requirements for the Position:

  • Valid Tennessee Driver’s License
  • Acceptable Motor Vehicle Report (MVR)
  • Acceptable Criminal Background Investigation
  • Personal Automobile Insurance
