COPE Community Services, Inc. is seeking an entry-level Behavioral Health Case Manager in Green Valley, AZ. The role focuses on providing face-to-face, phone, virtual, and in-home support to members on their recovery journey.

You will develop and update person-centered service plans, monitor progress, and ensure collaboration with primary care providers and other stakeholders. A strong emphasis on documentation and teamwork is required.

Qualifications

• Bachelor's or master's degree required or
• Associate degree with BH work experience acceptable
• HS diploma or GED with Peer Support Specialist cert plus BH experience
• HS diploma or GED with Medical Assistant cert plus BH experience
• HS diploma or GED with BH experience 3+ years preferred

Responsibilities

• Provide face-to-face, phone, virtual, and in-home services as needed.
• Develop, implement, and update person-centered individual service plans.
• Monitor progress and document service plan progress.
• Coordinate with primary care providers and other service providers.
