Job summary

Mercyhealth in Wisconsin seeks a Behavioral Health Case Manager BS - Outpatient to assess and assist with mental health and educational needs, coordinating care and advocating for patients and families within the interdisciplinary team.

You will join a 40-hour week, day shift, collaborating with physicians, preparing histories, discharge plans, and connecting patients with community resources, while pursuing Level 2 certification within 90 days after hire.

Qualifications

  • Bachelor's Degree in mental health or behavioral health field.
  • Tact Level 2 certification within 90 days after hire.
  • CPR/BLS certification preferred.
  • Ability to coordinate care and advocate for patients.
  • Experience working with interdisciplinary teams.

Responsibilities

  • Collaborates with physicians and health care staff to assess social and emotional factors.
  • Develops patient histories, treatment plans, and dispositional plans.
  • Coordinates care with relevant departments and interagency referrals.
  • Acts as a family liaison and primary contact for patients and families.
  • Maintains strong communication within the program and with psychiatrists.
  • Provides supervision to Social Work Students/Interns (as assigned).

OVERVIEWThe Behavioral Health Case Manager BS - Outpatient assesses and assists with the mental and psychiatric health and educational needs of patients by providing specialized services in order to meet identified patient or child and family needs. This role serves as the Patient and family's case worker, coordinates and facilitates program activities, interagency referral sources, acts as a community liaison, is a primary family contact person and promotes appropriate utilization of resources to contribute to improved patient satisfaction and outcomes. Specific fine motor abilities required include writing, keyboard entry and mouse use.LEVEL OF SUPERVISIONDirectly supervises the Social Work Students/Interns as assigned. Communicates the progress of each student on a regular basis to Team Leader. Would carry out supervisory responsibility in accordance with the organization's policies and applicable laws.SUPERVISESSupervised by Mercy Options Team leader.
