Merakey seeks a Blended Case Management Supervisor to guide BCM staff, oversee clinical and operational functions, and support individuals with serious mental illness in community settings. The role emphasizes high-quality, person-centered services, regulatory compliance, and partnership with families and providers.

Location-based in Monaca, PA with daily supervision, planning, and coordination responsibilities. Minimum pay starts at $48,000 per year, contingent on experience and qualifications.

Qualifications

• Bachelor's degree in sociology, social work, psychology, gerontology, anthropology, history, criminal justice, theology, counseling or education with five years of mental health care experience, including two in supervision.
• Registered Nurse (RN) with five years of direct mental health care experience, including two years in a supervisory role.
• Bachelor's degree in nursing with three years of direct mental health care experience.

Responsibilities

• Provide clinical and operational oversight of the Blended Case Management (BCM) program for high-quality, community-based care for individuals with SPMI.
• Supervise, coach, and support BCM staff through weekly clinical supervision, crisis guidance, problem solving, and onboarding.
• Lead service planning, admissions, referrals, discharge coordination, and treatment team meetings to promote continuity of care.
• Monitor documentation, productivity, and compliance to meet county, managed care, and state requirements and assist with program reports.
• Serve as a liaison with families, providers, and community partners to foster collaborative, recovery-oriented services.
