PruittHealth @ Home is hiring compassionate Home Health Registered Nurses (RNs) to provide skilled nursing visits in patients’ homes and to lead care plans. The role offers autonomy, flexible scheduling, and strong clinical support to help you deliver high-quality care.

As an RN Case Manager, you’ll collaborate with an interdisciplinary team, supervise aides, and use iPad-based documentation with AI assist. A driver’s license and CPR are required, with 1+ year RN experience preferred.

Qualifications

• Active, unrestricted RN license.
• Valid driver’s license and reliable transportation.
• CPR certification.
• 1+ year of RN experience (home health or acute care preferred).

Responsibilities

• Provide skilled nursing visits in the patient's home.
• Complete assessments, start-of-care visits, and care plans.
