Position Overview

The Home Health Registered Nurse uses an individualized patient care plan to provide top quality care to patients in their homes. This position is a Full‑Time Weekend (Friday, Saturday, Sunday) 12‑hour day shift. The role is based in Columbus and Bladen Counties and requires travel throughout the service area.

Responsibilities

• Conduct initial assessment and develop patient case plan for patients in all stages of life.
• Visit 6‑8 patients per shift and provide skilled nursing services.
• Assess, interpret, plan, implement, and evaluate patient care according to age and diagnosis.
• Document patient care in electronic medical records and respond to documentation requests via email during the week.
• Travel to patients' homes using company‑issued laptops.
