Job summary

Adoration Health in Bayonne, NJ is seeking a Home Health Physical Therapist for a PRN role with weekend hours (Saturday and Sunday, ~16 hours/week). You will deliver 1:1 patient care in home health, evaluate and treat patients per physician orders, and collaborate with a care team to support patient goals.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• New graduates may be considered in select markets.
• At least 2 years of experience as a physical therapist; hospice/community experience preferred.
• Valid license in the state of practice.
• Current CPR certification required.

Responsibilities

• Perform PT evaluation and treatment based on physician orders and adaptive equipment needs.
• Establish measurable goals and develop treatment plans to maximize independence.
• Maintain communication among treatment team, physician, patient, and family.
• Maintain thorough, timely documentation for accurate billing.
• Supervise PTAs and PT Aides in line with state practice act.

Responsibilities

As a Home Health Physical Therapist, You will:

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license form the state of practice
• Possess and maintains current CPR certification

Salary Range

USD $56.00 - $60.00 / Hour
