Job summary

WellSpan Health in York, PA seeks a Licensed Practical Nurse to coordinate treatment, evaluation and education for bariatric surgery patients in the York unit. This role emphasizes patient-centered care inside a dedicated bariatric surgery team.

You will serve as a primary contact for patients, arrange consultations, reinforce pre/post-procedure instructions and maintain clear communication with the care team to ensure safe, effective care.

Qualifications

• Certificate program post high school vocational/specialized training required.
• 2 years nursing experience required.

Responsibilities

• Provides initial contact with patients.
• Serves as a consistent contact for patient/family throughout the process of diagnosis and treatment.
• Coordinates appointments for consultations and physician visits.
• Notifies primary care and specialty care providers with changes in treatment protocol.
• Reinforces pre- and post-procedure instructions and provides further education as appropriate.
• Maintains two-way communication with providers’ offices to improve care coordination.
• Reports outcome of nursing/patient care/coordination activities to Service Line leadership.
• Protects patient confidentiality by promoting professional staff communications.
