Job summary

Hartford HealthCare at Home in Torrington, CT seeks a Registered Nurse for our Primary Care team. The RN will perform comprehensive assessments, develop care plans, and coordinate with the interdisciplinary team to support patients in their homes.

This full-time position requires CT RN licensure, at least one year of RN experience (homecare preferred), a valid driver’s license, and computer proficiency to document skilled services.

Qualifications

• Registered Nurse with Valid CT RN license.
• One year of RN experience; Homecare experience preferred.
• Valid driver’s license and reliable transportation required.
• Computer proficiency required.

Responsibilities

• Perform comprehensive assessments to establish an individualized care plan.
• Facilitate communication between the patient and the interdisciplinary team.
• Develop and maintain a therapeutic relationship by providing education and promoting self-advocacy and independence.
• Utilize state-of-the-art technology to document skilled services provided.
