FOX Rehabilitation is seeking licensed Occupational Therapists to deliver one-on-one care to older adults in private homes or senior living communities in Mequon, WI. You will conduct evaluations, tailor plans of care, and provide evidence-based therapy to improve mobility and independence.

Qualifikationen

• Valid Occupational Therapy license in the state, or eligibility to obtain degree from an accredited OT program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Aufgaben

• Provide one-on-one OT to older adults in private homes or senior living communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve wellness, mobility and independence.
• Document in FOX’s EMR system per regulatory requirements.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
