Job summary

Rehab Without Walls Neuro Rehabilitation, an affiliate of BrightSpring Health Services, is seeking a licensed Physical Therapist to provide functional rehabilitation in home and community settings across California. You will evaluate, treat, and advocate for clients in real-life contexts such as kitchens, grocery stores, workplaces, and outdoor environments.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practicing state.
• Current Basic Life Support or CPR certification per state regulations.
• Minimum of one year work experience as a PT preferred.
• Knowledge of rehabilitation techniques for complex neurological injuries.
• Effective verbal and written communication skills.
• Ability to lift 50 pounds and assist with patient transfers (up to 100 pounds).
• Duties require fine motor skills, visual acuity, and standing/walking for extended periods.
• Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning.
• Health screen may be required prior to assignment and periodically thereafter.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess treatment effectiveness and participate in staffing meetings.
• Conduct discharge evaluations and provide home/community safety recommendations.
• Communicate patient needs and progress to the treatment team, physician, and family.
• Collaborate with other disciplines to coordinate care and improve outcomes.
