Job summary

Iconic Care is hiring a full-time/contract Physiatrist to round in skilled nursing facilities and rehabilitation centers. Expect 1–3 locations, 25+ patients daily, and autonomous practice with clinical support as needed.

The role emphasizes joint injections, Botox, dry needling, and personalized care within multidisciplinary teams, with a strong focus on rehab outcomes and patient participation.

Qualifications

• Current, unrestricted PA or NP license.
• DEA licensed or willing to obtain DEA licensure.
• Nursing home and/or physiatry experience is a plus, but not required.
• Comfort performing peripheral joint injections (knees, shoulders, hips).
• Strong communication skills and ability to collaborate with multidisciplinary teams.
• Familiarity with EHR charting.

Responsibilities

• Rounding at 1–3 SNFs or rehab centers, 1–3 days per location, with 25+ patients per day.
• Perform focused assessments and create individualized plans with facility teams.
• Deliver injections, Botox, dry needling, and medication management as needed.
• Serve as sole on-site physiatrist, autonomous yet supported by a clinical team.
• Coordinate care and refer to other specialties as appropriate.
