Job summary

Marathon Health is seeking a Nurse Practitioner/Appropriate Provider to deliver relationship-based, holistic primary care in Fort Morgan, Colorado. You will provide prevention, chronic disease management, urgent care, and health education, practicing autonomously at the top of your license.

Active licensure with ANCC or AANP board certification is required along with CPR/BLS; a DEA number is preferred for full prescribing authority. Collaboration within a value-based care model is expected.

Qualifications

• Active license and ANCC or AANP board certification required.
• CPR/BLS certification required at start date.
• Independent practice provider preferred where applicable per State regulation.
• Federal DEA number preferred; may be required for full prescribing ability.
• Independent family practice experience including wellness care, chronic care management, and urgent visits preferred.
• Full Practice Authority strongly preferred where applicable per State guidelines.
• Ability to practice relationship-based medicine at the top of license.
• Willingness to provide prevention, chronic disease management, and health education.

Responsibilities

• Provide relationship-based, holistic primary care with prevention and chronic disease management.
• Deliver urgent care and health education while practicing autonomously at the top of license.
• Collaborate within Marathon Health’s value-based primary care model.
• Utilize UpToDate and RubiconMD to support clinical decisions.
