Job summary

Guardian Angel Home Care Inc. is seeking a compassionate Licensed Practical Nurse (LPN) to join our home health team in Fresno, CA.

You will provide skilled nursing care in patients' homes under the direction of the RN and physician, monitor patient condition, administer medications, and document care in HCHB. Qualifications include graduation from an accredited LPN program, current BLS/CPR, valid driver’s license, and strong communication and documentation skills.

Qualifications

• Graduate of an accredited practical nursing program.
• Current BLS/CPR Certification.
• Valid driver's license and reliable transportation.
• Strong clinical, communication, and documentation skills.
• Ability to work independently in a home health setting.

Responsibilities

• Provide skilled nursing care in patients' homes under the direction of the RN and physician.
• Monitor patient condition, vital signs, and overall health status.
• Administer medications and treatments as ordered within the LPN scope.
• Document visits and care in Homecare Homebase (HCHB).
• Communicate changes in patient condition to supervising RN and physician.
• Educate patients and families on medications, disease management, and plan of care.
• Ensure compliance with federal, state, and agency regulations.
• Collaborate with interdisciplinary team to achieve optimal outcomes.
