Qualifications

• Valid Physical Therapy license in the state of practice or eligibility to obtain.
• Basic computer and EMR proficiency.
• Current CPR certification.

• Perform evaluations and deliver evidence-based treatment.
• Document effectively within FOX’s EMR system.
