VitalCaring is seeking an experienced Home Health RN to lead patient care in home settings, manage plans of care, and coordinate across disciplines. You will drive outcomes from admission to discharge with advanced assessment, care planning, and patient education.

You will deliver skilled nursing care, supervise aides, and participate in on-call rotations to ensure high-quality, patient-centered outcomes while maintaining strong documentation and independent judgment.

Qualifikationen

• Active RN license (or compact eligibility) and CPR certification.
• Minimum 1 year RN experience; Home Health, Med-Surg, ICU, or Acute Care preferred.
• Proficient with EMR systems and able to manage a caseload.

Aufgaben

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care
• Develop and manage individualized care plans with physicians and teams
• Deliver skilled nursing care including medications, treatments, and interventions
• Act as case manager coordinating across therapists, aides, and physicians
• Monitor patient progress and adjust care based on changes in condition
• Educate patients and families to support disease management and independence
• Coordinate transitions and discharge planning to reduce readmissions
• Maintain timely documentation in the EMR
• Supervise Home Health Aides as needed
• Participate in on-call rotation as appropriate
