Job summary

Canyon-Home-Care-and-Hospice-1 is seeking a Registered Nurse to administer skilled nursing care in patients' homes under physician orders and the plan of care. You will assess, implement, and coordinate care with the IDT to meet patient needs.

Responsibilities include initiating and revising care plans, documenting care in a timely manner, coordinating with physicians and therapists, and ensuring quality and compliance with agency policies.

Qualifications

• Graduate of an approved school of professional nursing and licensed in the state(s) where practicing.
• 1 year nursing experience preferred.

Responsibilities

• Provides services per the plan of care and initial evaluation visit.
• Reassesses nursing needs and revises the care plan with IDT involvement.
• Completes OASIS assessments and documentation on a timely basis.
• Maintains confidentiality and adheres to agency policies.
• On-call duties nights, weekends, and holidays as assigned.
