Riverside Home Health Care and Hospice in Southern Oregon seeks a dedicated Physical Therapist to provide skilled care in patients’ homes across Medford and Grants Pass. You will develop individualized care plans, perform admissions and routine visits, and collaborate with a multidisciplinary team to ensure compassionate, ethical, and patient-centered outcomes.

Qualifications

• Oregon PT license (or ability to obtain before start date).
• 1–2 years PT experience (home health preferred, not required).
• Current CPR certification.
• New PT graduates encouraged to apply.

Responsibilities

• Providing skilled PT care in patients’ homes

• Developing and carrying out individualized care plans
• Collaborating with nurses, therapists, and providers
