Qualifications

Current and unrestricted LPN licensure in state of practice.
Current driver's license and vehicle insurance or access to dependable transportation.
Preferred CPR certification or ability to obtain within 90 days.
Home care experience
Ability to manage multiple tasks and work independently.
Good communication, writing, and organizational skills.
Ability to work flexible hours to meet client needs.

Responsibilities

Provides high quality clinical services within the scope of practice as defined by state laws and plan of care.
Documents patient visits thoroughly in the electronic medical record.
Participates in care coordination and reports changes in condition.
