Qualifications

• High school diploma or equivalent required.

Responsibilities

• Encourage independence and community participation.
• Assist with hygiene, grooming, and meal planning/preparation.
• Provide transportation and support for community activities and employment.
• Maintain professional interactions with individuals and families; coordinate services.
• Administer medications per policy and attend medical appointments.
• Participate in ISPs design and plan meetings.
