Job summary

Theoria Medical is seeking a PRN Attending Physician to serve local skilled nursing facilities within a mission-driven care community. You will build lasting trust with residents, providing continuity of care beyond quick visits.

Qualifications

• Active U.S. medical license
• Board-certified or board-eligible in Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, or Geriatric Medicine

Responsibilities

• Serve as the attending physician for patients in each assigned SNF
• Collaborate with NPs/PAs to manage daily patient care and supervise clinicians
• Provide high-quality, patient-centered primary care, diagnosing and treating acute and chronic conditions
• Participate in Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives
