Job summary

Community HCS is seeking dependable, compassionate Personal Care Aides in San Antonio, TX to assist clients in their homes with daily living activities and essential health support. You will follow detailed care plans, monitor clients, document observations, and communicate clearly with supervisors.

A valid driver’s license and reliable transportation are required if travel is involved. High school diploma or GED is preferred.

Qualifications

• High school diploma or GED; preferred but not required.
• Previous experience as a PCA, caregiver, or in related field preferred.
• Ability to follow care plans and maintain clear communication.

Responsibilities

• Provide personal care such as bathing, oral care, and dressing.
• Assist with meal preparation, serving food, and light housekeeping.
• Perform and record vital signs and intake/output as directed.
• Observe safety hazards and report findings to supervisor/RN.
• Attend required in-services and orientation.

Responsibilities/essential functions: The person in this position must be able to perform the following essential job functions with or without reasonable accommodations.Follows the instructions of the PAS Supervisor in providing personal assistance services.*Provide assistance with hygiene such as bathing, oral care, and dressing.*Assist with food preparation; serve food, performs errands, light house keeping, laundry, wash dishes.*Perform and record accurate measurements (i.e. vital signs, or intake/output as instructed in the care plans). *Observe and report any safety hazards found in the clients home or any significant observations regarding the client.*Attend staff meetings, attend/complete 12 hours of annual in-services, and participates in orientation of new employees. *Report client complaints to the PAS Supervisor. *Maintain client confidentiality/adheres to HIPAA requirements and agency policy and procedures manual. *Possess the ability to follow written and oral instructions. *Report abnormal findings in clients conditions as observed and per care plan to RN. *The above list reflects the essential functions and other job functions considered necessary of the job identified, and shall not be construed as a detailed description of all work requirements that may be inherent in the job, or assigned by supervisory personnel.
