Cityblock Health in Worcester, MA is seeking an Associate, Care Team Operations to support administrative tasks and follow up for connecting members to needed services under the direction of Community Health Partners and Nurse Care Managers. This is an in-person role located in Worcester, MA.

The position involves coordinating services, initiating referrals, communicating with insurance and providers, and supporting care teams with documentation and reporting to improve member outcomes.

Qualifications

• 1+ year of experience in an administrative role, managing scheduling and coordination tasks (appointments, follow-ups, or care plans).
• Experience in maintaining accurate records and documentation.

Job Description: The Associate, Care Team Operations plays a key role in supporting members who we see clinically in social and care management needs. This position serves as support in administrative tasks and follow up for connecting members to their needs, by direction of the assigned Community Health Partners and Nurse Care Managers to the member. Requirements

• 1+ year of experience in an administrative role, managing scheduling and coordination tasks (appointments, follow-ups, or care plans); and/or experience in maintaining accurate records and documentation.

• This role is eligible for health insurance, life insurance, retirement benefits, participation in the company’s equity program, paid time off, including vacation and sick leave.
• The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location.
• The expected salary range for this position is: $20.19 - $25.96 Hourly.
