Responsibilities
  1. Ensure health and safety of individuals, including personal care.
  2. Provide education, training, counseling, and assistance to achieve ISP goals.
  3. Communicate with families, guardians, advocates, and community stakeholders.
  4. Develop and present ISP plans and agreements.
  5. Perform case management duties.
  6. Provide transportation as needed.
  7. Supervise and schedule staff.

Requirements
  • BA/BS in Human Services or related field, or 10 years of relevant experience.

  • Ability to complete Lifeworks training including CPR, First Aid, and Medication Certification.
  • Pass background checks including CORI, reference, and driving record.
