Job summary

Miami Jewish Health is hiring an Assistant Case Manager to support Medicare/Commercial Advantage plans. You will coordinate medical appointments and provide clinical updates to managed care agencies, ensuring smooth transitions from admission to discharge.

The role requires organizing follow-ups, coordinating equipment and services for discharges, and maintaining bed management oversight while protecting patient confidentiality. Strong English/Spanish communication is essential.

Qualifications

• High School Diploma or equivalent; Associate degree preferred.
• One-year experience in a community service agency that provides service to elderly clients preferred.
• Bi-lingual English/Spanish required.
• Good communication skills and confidentiality emphasized.

Responsibilities

• Review all Medicare Advantage admissions and discharges daily.
• Assist case manager in coordinating follow-up medical appointments.
• Review pending medical appointments and follow up with nursing units and transportation.
• Provide clinical updates and documentation requested by Managed Care agencies.
• Coordinate equipment and services for pending discharges.
• Provide bed management oversight and other duties as needed.

Job Title: Assistant Case Manager

Job Summary

This is a paraprofessional position that performs a variety of case management duties for Medicare/Commercial Advantage Plans. Job functions include coordinating medical appointments and providing managed care agencies with clinical updates as requested.

Essential Job Functions

• Review all Medicare Advantage (Managed Care/HMO) admissions and discharges daily.
• Assist case manager in coordinating follow up medical appointments
• Review all pending medical appointments and follow up with nursing units and transportation department.
• Provide clinical updates and other documentation requested by Managed Care agencies.
• Coordinate equipment and services for pending discharges.
• Provide bed management oversight, and other duties as needed

Job Requirements

• High School Diploma or equivalent. Associate degree preferred.

Experience

Abilities Required

• Sensitivity and skill required in working with impaired elders and families by phone
• Must be organized, able to work independently and understand confidentiality which is necessary for this program.
