Job summary

Generations Assisted Living & Memory Care in North Carolina seeks a compassionate aide to coordinate transportation for residents' medical appointments and provide personalized care. You will support daily living activities, accompany residents to appointments, and communicate with the care team.

This role helps uphold the facility's memory care philosophy and promote resident well-being. The position requires Nurse Aide 1 or willingness to train, and experience with older adults.

Qualifications

Job description

Position Summary:

Provide, schedule and coordinate transportation for residents’ necessary medical appointments.Provide personalized care and service, uphold the philosophy of assisted living, and promote the well-being of the residents.Provide assistance and support with residents’ activities of daily living, and instrumental activities of daily living.Provide support to the Resident Care Team.


Primary Responsibilities:

Transportation Services:


  1. Assist residents and families in scheduling necessary medical appointments.

  2. Maintain an appointment calendar and update as necessary.

  3. Confirm upcoming appointments on a daily basis with family and Dr.’s office.Reschedule if needed.

  4. Prepare needed paperwork in advance, to be taken with the resident to each appointment.Necessary paperwork shall include but not be limited to: list of current medications (MAR), copy of all insurance cards, necessary progress notes, physician visit notes, and physician order sheet.More or less paperwork may be required based on the nature of the physician visit.

  5. Transport residents to medical appointments via facility/agency vehicles.Memory Care residents should not be left unattended at any time.Additional support staff needed for appointments will be pre-approved by supervisor as needed.

  6. Accompany residents while in the Dr.’s office.Assist the resident in communicating concerns or problems to the Dr.Also communicate to the Dr. any concerns of the facility via the Physician/Facility Communication Form, if the resident is unable to voice concerns.

  7. Relay all instructions from the Dr., new orders, new medications, and follow-up visits to the respective Care Coordinator and med-aide on duty.

  8. Document in residents’ charts the record of visits and reasons for visits.

  9. Provide weekly appointment list to the Care Coordinators.Discuss need for appointments with Care Coordinators.


Support Services


  1. Keep medication rooms and files rooms clean and organized.

  2. Transport documents to/from physician’s offices.Assist with picking up medications, delivering specimens to lab and/or doctor’s offices.

  3. Obtain needed hard scripts and prior authorizations for resident’s medications.

  4. Thin current resident charts.Break down charts after resident is discharged.Maintain proper filing and storage of charts per regulations.

  5. Clean glucometers on a weekly basis.

  6. Inventory test strips and diabetic supplies each week.Ensure residents have adequate supplies at all times.

  7. Prepare Physician Orders and Standing Orders to send to physicians every 6 months.

  8. Check hearing aids/coordinate hearing aid needs with appropriate providers on a monthly basis.

  9. Make copies of necessary forms and keep file drawers supplied.

  10. Conduct a monthly alarm and wander guard bracelet audit.

  11. Inventory incontinence supplies on a monthly basis.

  12. Obtain monthly vital signs as needed and as directed.

  13. Provide transportation each week for a resident shopping day.

  14. Print monthly MARs/TARs and file.

  15. Complete weekly cart audits

  16. Function as part of management team (manager on duty coverage, on-call scheduling, marketing the facility, planning and participating in staff meetings and facility events).


Resident Services


  1. Assist with Resident Assistant job duties as needed and as directed by supervisor.

  2. Assist with Medication Aide duties as needed and as directed by supervisor.


Communications


  1. Attend all regular staff meetings and required training sessions.

  2. Effectively communicate information pertaining to the residents to other team members according to the residence policies and procedures.

  3. Notify the Memory Care Coordinator or Resident Care Coordinator if you observe a change in a resident’s condition.

  4. Respond to on-the-job injuries in accordance with the facility’s policies and procedures.

  5. Notify your supervisor of any concerns or questions presented by the residents’ family or residents.

  6. Assist with answering telephone, and directing calls to appropriate staff.

Physical Job Requirements

To fulfill job requirements, employee must be able to perform key physical tasks for extended periods of time.When supporting, lifting, or transferring residents, proper body mechanics and safety measures as prescribed by the residence must be used.


In a typical eight hour shift the employee may be asked to:



  • Stand/walk up to 8 hours per day

  • Sit up to 2 hours per day

  • Frequently support up to 70 pounds

  • Occasionally lift/carry up to 15 pounds

  • Frequently kneel, bend, reach and carry

  • Secure proper assistance for transferring and lifting of residents as needed.
