Cityblock Health seeks an experienced RN to perform field-based hybrid assessments and care planning for members in Worcester, MA. The role involves in-person visits up to ~80% and remote work the rest of the time, with a focus on comprehensive assessments, medication reconciliation, and preventive care within a value-based model.
Required is an RN license with 3+ years of experience, strong clinical judgment and motivational interviewing skills.
Work Type: Field-Based Hybrid (up to approximately 80% in-person/community-based work, generally averaging approximately three member assessment visits per day based on member and business needs, with remaining work completed virtually/remotely subject to change) Travel: Mileage for approved business travel is reimbursed in accordance with company policy. The Assessment RN completes MDS and relevant screenings within assigned territory as scheduled to capture members’ clinical needs and meet regulatory requirements, and also works with members to create a comprehensive care plan that accurately reflects members’ needs and healthcare goals. Uses clinical judgment to elevate members with urgent clinical needs to appropriate departments as needed.
