The Assessment Coordinator is a patient‑centered role supporting patients and their families during some of their most important moments. This position serves as the first point of contact for individuals ages 5–65+, conducting compassionate interviews and clinically informed assessments to determine the best path forward for care. The Assessment Coordinator collaborates closely with our Business Office to ensure smooth financial onboarding and partners with Utilization Review to secure certifications for admitted patients.
