Job summary

Responsibilities

  • Maintain expertise in drug therapy across all patient age groups.
  • Collaborate with physicians to review medication histories, educate patients, establish therapeutic goals, assess demographics, develop and implement treatment plans, monitor outcomes, and document medication-related issues.
  • Evaluate drug therapy for appropriateness, dosage, regimen, and potential interactions.
  • Precept pharmacy students and interns in alignment with academic credentials and clinical experience.
  • Conduct drug usage evaluations and report findings to relevant committees.
  • Detect, document, and report adverse drug reactions and medication errors.
  • Promote formulary use and coordinate procurement of non-formulary medications when necessary.
  • Counsel patients on discharge medications as needed.
  • Participate in emergency drug therapy management, including responding to Code Blue situations.
  • Maintain and update patient profiles with relevant clinical information.
  • Clarify medication orders with prescribers and ensure accurate documentation and communication.
  • Reconcile pharmacy and nursing medication records daily.
  • Verify accuracy of medication batches and orders prepared by technicians.
  • Provide drug information to healthcare professionals and patients.
  • Deliver presentations and publish drug-related educational materials.
  • Identify cost‑saving opportunities in drug therapy and document clinical interventions.
  • Ensure proper preparation and timely dispensing of pharmaceutical products, including oral, IV admixtures, chemotherapeutic, and investigational agents.
  • Comply with controlled substance distribution and control regulations.
  • Adapt to various pharmacy areas as workload demands.
