Qualifications

• Current, unrestricted Arizona Nurse Practitioner license is required.
• National FNP certification is required.
• Family medicine or primary care experience is preferred.
• Must be willing to provide coverage at either the Central Valley or West Valley clinic.

Responsibilities

• Provide comprehensive primary and family medicine care to patients across the lifespan.
• Evaluate, diagnose, and treat acute and chronic medical conditions.
• Perform physical examinations, health assessments, and preventive care.
• Develop and manage individualized treatment plans.
• Order and interpret diagnostic testing as appropriate.
• Prescribe medications and provide medication management.
• Provide patient education regarding preventive health, wellness, and chronic disease management.
• Coordinate referrals and follow-up care with specialists and other healthcare professionals.
• Maintain accurate and timely clinical documentation.
