Brookdale is a leading elder care provider with a nationwide footprint. We are seeking an RN with clinical leadership experience to travel weekly across districts, provide clinical support, ensure regulatory compliance, and lead training initiatives for clinicians in Brookdale communities.

You will audit, analyze root causes, and help implement action plans to improve patient outcomes while collaborating with district leadership. A valid RN license and driver’s license are required.

Qualifications

  • Associate’s degree in nursing or Diploma RN required with 2+ years of direct nursing experience with older adults.
  • Current RN license; valid driver’s license and access to a private vehicle for business use.
  • Ability to supervise associates who pass medications and to apply guidelines to daily decisions.

Responsibilities

  • Travels throughout the district weekly to communities as assigned.
  • Fills the role of HWD when on leave or vacancy; provide clinical support and guidance for communities with regulatory deficiencies.
  • Audit community clinical compliance and help design and implement action plans.
  • Provide root cause analysis to ensure clinical outcomes meet standards; support service plans and PCC compliance.
  • Lead district and community training; coordinate RN delegations and medication administration as required.
  • Participate in meetings; support clinical initiatives and QA site visits.
  • Provide SWOT insights and regular reports for the DDCS; assist in special projects.

Base pay in range will be determined by applicant’s skills and experience. Full-time associates in role are also eligible for an annual bonus incentive. Temporary associates are not benefits eligible but may participate in the company’s 401(k) program.

Responsibilities
  • Travels throughout the district weekly to communities as assigned by the DDCS.
  • Fills the role of HWD in assigned communities when an HWD is on leave, extended vacation or the position is vacant.
  • Provides clinical support and guidance for communities with regulatory deficiencies.
  • Audits community clinical compliance and partners with other leaders to design and implement action plans as needed.
  • Provides root cause analysis and hands on support to ensure clinical outcome are meeting standards and key areas are current, which may include: service plans, orders, PCC compliance, MAR clarification, weights, skins, etc. Ensures compliance post-cleanup as needed.
  • Supports clinical initiatives and roll-outs by leading training for the district and/or individual communities as assigned by DDCS.
  • In some states coordinates and signs resident assessments. This may include completing comprehensive assessments including personal interview with the resident and other if the resident due to any impairment needs assist with history, etc.
  • Facilitates RN Delegations for medications and care per state regulations.
  • Participates in a variety of meetings with the division, district and individual communities.
  • Provides training, coaching and guidance to clinical leaders and front line staff.
  • Participates in clinical training and audits compliance; including facilitating Med Tech/QMAP/Med. Aide training and competency evaluations.
  • May assist DDCS in clinical Quality Assurance (QA) site visits.
  • Provides insight to DDCS on overall assessment of community strengths, weaknesses, opportunities, and threats (SWOT). May be asked to provide regular reports.
  • Provides support in clinical investigations and may participate in disciplinary actions.
  • Supports DDCS with special projects as assigned.
  • Provide district clinical support when DDCS is unavailable which may include responding to questions, support move in/out discussions etc.
  • Ensures proper and timely documentation in accordance with Brookdale policies and procedures and federal and state requirements.
  • Maintains a working knowledge of and adheres to company policies, procedures and Clinical Systems.
  • Utilizes the nursing process (assessment, planning, implementation, and evaluation) in applying nursing knowledge in accordance with state nursing regulations.
Qualifications

Education and Experience
Associate’s degree in nursing, health education or related field or Diploma RN. Two or more years of direct nursing experience with older adults.

Certifications, Licenses, And Other Special Requirements
Current nurse (RN) license, issued by appropriate state licensing agency. Must have a valid driver’s license and access to a private vehicle for business use.

Management/Decision Making
Applies existing guidelines and procedures to make varied decisions within a department. Uses sound judgment and experience to solve moderately complex problems based on precedent, example, reasonableness or a combination of these. If serving in single community, may directly supervise associates who are certified or trained to pass medications.

Knowledge And Skills

Possesses extensive knowledge of a distinct skill or function and a thorough understanding of the organization and work environment. Has working knowledge of a functional discipline. Basic typing skills along with a working knowledge of personal computing and word processing software are required, preferably in a Microsoft Windows environment. Ability to read and interpret documents. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or associates of company. Ability to effectively communicate in English and understand and follow written and oral instructions.
