Qualifications

• 2+ years of experience as a provider of Specialty care.
• Experience managing chronic disease populations is preferable.

Responsibilities

• Works in collaboration with a supervising physician.
• Manages a panel of patients and supports appropriate interventions.
• Participates in Quality Improvement practices in the clinic.
