GENERAL DESCRIPTION

This position is part of the medical clinical team. This position is responsible for providing primary care and/or walk in care services, including assessing and managing patients in the clinic following established standards of practice. This position shall be authorized to make independent decisions about advanced practice clinical needs of families, patients and medical decisions based on scope of practice. Providers working in Oklahoma facilities will practice under the authorization of collaborative practice with one or more physicians.

Requirements

ESSENTIAL DUTIES

  1. Develops and manages the medical plan of care for patients based upon an agreement developed jointly and signed by the mid-level and supervising physicians.
  2. Provides complete, comprehensive outpatient care for patients including health examinations and treatment of medical conditions, to include follow-up of problems detected.
  3. Responsible to manage acute and chronic illness, routine care through care plans that are coordinated, when appropriate, with the patient's behavioral health provider
  4. Responsible to make appropriate referrals to internal disciplines of CHC/SEK and/or external resources/agencies for additional services required.
  5. Prescribes medications for patients for treatment of conditions, as indicated.
  6. Provides clinical consultation to other providers within CHC/SEK as needed.
  7. Responsible for accurate, timely, and clear medical record documentation as defined by the Electronic Health Record Completion Policy.
  8. Observes clinic protocols and guidelines and scope of practice as determined by professional regulatory agencies.
  9. Provide appropriate follow-up with clinical support staff to include telephone/written correspondence (e.g. inform patients of test results).
  10. Provides clear written and oral communication to patients/guardians, and employees.
  11. Participates in an on-call system for after-hours care for CHC/SEK inpatient population, as assigned.
  12. Participates in quality improvement activities.
  13. Ability to weigh the relative costs and benefits of a potential action.
  14. Supervises healthcare students as assigned.
  15. Floats to alternate CHC/SEK facilities as needed to ensure appropriate access to care.

QUALIFICATIONS

  1. If working in Kansas, valid Kansas Nurse Practitioner license.
  2. If working in Oklahoma or Columbus WIC, valid Oklahoma Nurse Practitioner license with prescriptive authority required. Oklahoma Nurse Practitioner license may be required by other APRN’s, depending on location and practice.
  3. Valid Kansas Registered Nurse license or multistate license in nurse’s resident state. An Oklahoma Registered Nurse license may be required, if no multistate license.
  4. Graduate of an accredited Advanced Nurse Practitioner program.
  5. Current board certification through ANCC or AANP.
  6. Completion of one-year experience in clinic setting, preferred.
  7. If position/practice requires, must obtain DOT Certification within 30 days of hire and recertify every 10 years.
  8. ACLS required for Pleasanton providers and WIC providers in Fort Scott and Pittsburg.

  5. Must be computer literate and able to navigate the Electronic Health Record.
  6. Must be able to maintain good inter-personal relationship with co-workers and other members of the health care team and the organization.
  7. Provide customer service in accordance to the organization’s mission.
  8. Be courteous and respectful when interacting with patients and family members.
  9. Maintain patient confidentiality in accordance to organization’s policy and procedure and HIPAA requirements.
  10. Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals.
  11. A strong community/public health orientation, preferred.
  12. Commitment to serving the underserved.

WORK ENVIRONMENT

While performing the duties of this Job, the employee is regularly required to sit and/or stand; use hands to finger, handle, or feel; reach with hands and arms and talk or hear. The employee must occasionally lift and/or move up to 10 pounds. Specific vision abilities required by this job include close vision, peripheral vision, depth perception and ability to adjust focus.
