Job summary

The HF Coordinator - APRN will work as part of the heart care/EP team to coordinate care in a timely fashion to improve patient outcomes.

The coordinator will monitor suspicious imaging findings on routine and screening studies as well as follow positive pathology results and work with physician team to ensure appropriate and timely follow up studies and treatment are received. The HF Coordinator will provide survivorship care planning and assist with referrals to support services as needed.

Qualifications

• APRN credential and specialized training in chronic cardiovascular care.
• Experience coordinating patient care in a cardiology/EP setting preferred.
• Strong communication and organizational skills required.

Responsibilities

• Coordinate care within the heart care/EP team to improve patient outcomes.
• Monitor suspicious imaging findings from routine studies and ensure timely follow-up.
• Communicate with patients and follow care pathways throughout treatment.
• Provide a survivorship care plan upon graduation and coordinate referrals to support services.

JOB REQUIREMENTS

Education

• APRN with experience in chronic care and heart failure, EP, or equivalent.

Experience

• Must have 3 plus years as a APRN or 5 plus years as a RN in chronic cardiovascular setting.

Physical

• Normal hospital environment. Hearing of normal and soft tones. Pleasant voice, self-starter, well organized, able to work with minimal supervision. Comfortable working in a confined office environment. Lifting up to 20lbs, Carrying up to 20 lbs, Pushing/Pulling 250 lbs. Frequent sitting, standing, walking, bending and computer work. Capable of handling several tasks at one time. Close eye work. Uses computer, copier, typewriter, and fax machine.

JOB SUMMARY

The HF Coordinator - APRN will work as part of the heart care/EP team to help coordinate care in a timely fashion to improve patient outcomes. Coordinator will monitor and track suspicious imaging findings on routine and screening studies as well as follow all positive pathology results and work with physician team to ensure appropriate and timely follow up studies and treatment are received. HF Coordinator will be responsible for communicating with patients and following care throughout their treatment as well as providing a survivorship care plan upon graduation. The HF Coordinator will work closely with heart care team to ensure psychosocial needs are addressed and will assist with referrals to support services if needed.
