Job summary

Yale New Haven Health System is seeking an APRN or PA to join a stable, collaborative practice in the Mystic, CT area. You will provide comprehensive care in a team-based environment, leveraging EPIC EMR and working with a broad referral network across the Yale New Haven Health system.

Qualifications

  • Graduate from an accredited Nurse Practitioner or Physician Assistant program.
  • Clinical working experience as NP/PA preferred but not required.
  • Active PA Certification by the NCCPA, CT State Physician Assistant License, ACLS/BLS certifications when indicated, and DEA license if required.

Responsibilities

  • Evaluate patients and treat acute and chronic illnesses.
  • Orders, performs and interprets diagnostic tests.
  • Involves patients and their families in care decisions and treatment plans.
  • Creates treatment plans and completes orders in patient charts.
  • Collaborates with staff and other physicians to improve care delivery.
  • Initiates appropriate specialty referrals and provides medical information to the interdisciplinary team.
  • Ensures timely and accurate patient documentation; supports billing and coding processes.
  • Works with practice management to ensure compliance with CPT coding.
  • Maintains optimal patient access and participates in protocol development.

Education

Graduate from an accredited Nurse Practitioner or an accredited Physician Assistant Program.

Responsibilities

  • Initiates appropriate specialty referrals
  • Provides clinical information to the interdisciplinary team in a clear and timely manner
  • Ensures that proper patient documentation is timely, accurate and complete
  • Works with practice management to ensure proper compliance with billing, coding and CPT code assignment
  • Achieves and maintains optimal patient access
  • Provides interdepartmental medical consultation as requested
  • Participates in the development of medical protocols, standard procedures, and management of patients
Qualifications
Education
  • Graduate from an accredited Nurse Practitioner or an accredited Physician Assistant Program.
Experience
  • Clinical working experience as an NP/PA in the specialty/service line is preferred but not required.
  • APRNs, PAs, CRNAs, and CNMs must be appointed to the Affiliated Medical Staff of a YNHHS hospital and go through the Medical Staff credentialing process. To be eligible, evidence of current competence to practice as an APRN, PA, CRNA, or CNM must be provided. This includes obtaining appropriate reference letters from physicians and other practitioners during the Medical Staff appointment process.
Licensure
  • Active PA Certification by the National Commission of Certification of Physician Assistants (NCCPA), Connecticut State Physician Assistant License, ACLS/BLS Certified, when indicated for position profile DEA License.
  • Pediatric position profile PALS, when indicated for neonatal care NRP Certification.
  • Additional licensure, certifications and/or continuing education credits may be required during employment based on position and area of specialty. OR Connecticut State Registered Nurse License, Connecticut State Nurse Practitioner License, Board specialty certification, as appropriate for position profile ACLS/BLS Certified, when indicated for position profile DEA License.
