Job summary

Sanford Health in Minnesota seeks a Family Medicine Nurse Practitioner to provide advanced nursing care to patients of all ages, including assessing, diagnosing, and managing illnesses in collaboration with physicians.

You will perform exams, obtain histories, prescribe within scope, develop care plans, educate patients, coordinate referrals, and participate in quality improvement in a team-based primary care practice. This is a full-time daytime schedule.

Qualifications

• Master's or Doctorate degree from an accredited Nurse Practitioner program.
• Experience in primary care or family medicine preferred.
• Current licensure as a Registered Nurse and Nurse Practitioner in each state of practice.
• National board certification as a Family (FNP-C) or (FNP-BC).

Responsibilities

• Conduct comprehensive physical exams and health assessments.
• Obtain and interpret patient histories and diagnostic tests.
• Diagnose and treat common acute and chronic medical conditions.
• Prescribe medications and therapeutic treatments within the scope of practice.
• Develop and implement individualized care plans.
• Provide health education and counseling on disease prevention and healthy lifestyle choices.
• Manage follow-up care and coordinate referrals to specialists when needed.
• Perform minor procedures as appropriate (e.g., wound care, suturing).
• Document patient care accurately in the electronic health record system.
• Collaborate with physicians, nurses, and other healthcare team members.
• Maintain current knowledge of evidence-based practice guidelines.

• Experience in primary care or family medicine preferred
• Current licensure as a Registered Nurse and Nurse Practitioner in each state of practice.
• Current Drug Enforcement Administration (DEA) permit to prescribe controlled substances in each state of practice.
• State Controlled Substance License (if required by State)
• National board certification as a Family (FNP-C) or (FNP-BC)
• Obtains and subsequently maintains required specific competencies and certifications as outlined on privilege form.
