Job description

The Advanced Practice Professional (APP) provides high-quality,patient-centered care as a member of the interdisciplinaryhealthcare team. Working collaboratively with physicians, nurses,and other healthcare professionals, the APP assesses, diagnoses,treats, and manages patients within their scope of practice andarea of specialty. The APP is responsible for deliveringevidence-based care, promoting positive patient outcomes,maintaining professional and regulatory standards, and supportingthe clinical, educational, and operational goals of the departmentand organization.

Additional Job Description:

The Advanced Practice Professional (APP)is an individual with clinical skills and medical skills, with anexpertise in telemedicine. The APP completes comprehensive patienthealth evaluations (including biological, social and psychologicalassessments, co-morbid conditions, overall disposition, andguardian(s)); in collaboration with the appropriateclinical/faculty team. The APP assesses, and coordinates theeducational and psychosocial needs of the patient and caretakersvia telemedicine technology, taking in account the privacy of thepatient as it relates to theparent(s), guardians, and thedesignated caregivers. The APP develops and reviews the planofcare with the other members of the clinical team, participates indirecting that care, and coordinates discharge planning/and orfollow-up (as applicable). The APP participates in the education ofresidents and medical/nursing students and performs diagnosticprocedures and therapeutic procedures.

Provide healthcare services consistent with state law and therequirements of the professional licensing or certificationauthority and/or hospital privileges, where applicable. Suchservice may include, but are not limited to.

Note: Provides onsite clinical services to patients at J.W. RubyMemorial Hospital and supports affiliated system hospitals throughtelemedicine.

MINIMUM QUALIFICATIONS FOR NURSE PRACTITIONER :
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
  • 1.Graduate of a CCNE accredited nursing program with Master’sDegree in Nursing.
  • 2.National Certification by one of the recognized APRNCertification Centers.
  • 3.Licensure or eligibility for licensure as a professionalRegistered Nurse and Advanced Practice Registered Nurse in WestVirginia.
  • 4.American Heart Association BLS Certification required withadditional Advanced Life Support training at the discretion ofthe
    department.
EXPERIENCE:
  • 1.Telemedicine experience preferred.
  • 2.Eligible new graduates will be considered.
PREFERRED QUALIFICATIONS :
  • 1.PMHNPs preferred.
  • 2.FNP and ACNP certifications considered.
MINIMUM QUALIFICATIONS FOR PHYSICIAN ASSISTANT :
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
  • 1.Bachelor’s Degree as Physician Assistant or Master’s Degree asPhysician Assistant required.
  • 2.Graduate of an ARC-PA approved Physician Assistant (PA) trainingprogram required.
  • 3.Certification by the National Commission for Certification ofPhysician Assistants required.
  • 4.Must be licensed or eligible for licensure by the WV Board ofMedicine or WV Board of Osteopathic Medicine.
  • 5.American Heart Association BLS Certification required withadditional Advanced Life Support training at the discretion ofthe
    department.
EXPERIENCE:
  • 1.Telemedicine and/or psychiatry experience preferred.
  • 2.Eligible new graduates will be considered.
PREFERRED QUALIFICATIONS :
  • 1.Master’s Degree as Physician Assistant preferred.
  • 2.Psychiatry CAQ preferred.
CORE DUTIES AND RESPONSIBILITIES:
  • 1.Manages comprehensive and individualized care for patients andtheir families via telemedicine technology.
  • 2.Completes Emergency Room telepsychiatry consults.
  • 3.Completes integrated assessments; communicating the informationwith the clinical team in the Department of BehavioralMedicine.
  • 4.With the collaboration of designated faculty or by followingestablished clinical Guidelines and Protocols, orders additionaltests or consults as deemed appropriate.
  • 5.Performs and communicates results of diagnostic tests followingestablished clinical guidelines and scope of practice.
  • 6.Consults and communicates with clinic and hospital staffregarding abnormal findings in the evaluations of patients.
  • 7.Performs succinct comprehensive clinical evaluations viatelemedicine technology and documents findings in medicalrecord.
  • 8.Facilitates patient discharge and or follow-up with appropriateservices for telemedicine patients necessary for outpatientfollow-up appointments in the clinic (as applicable).
  • 9.Develops a comprehensive individualized plan of care forpatients, which includes teaching, discharge planning and necessaryreferral to other members of the health care team asnecessary.
  • 10.Demonstrates expertise in caring for patients across the agecontinuum of their educational experience and certification.
  • 11.Identifies unstable or complex patient care situations andfacilitates appropriate intervention in the clinic and/or hospitalsetting.
  • 12.Recognizes emergency situations and initiates effectiveemergency care.
  • 13.Evaluates patients and serves as a coordinator for follow upprocedures and tests in the ambulatory setting or admission forfurther diagnostic and therapeutic intervention (asapplicable).
  • 14.Assists in the collection of outcome measures (asapplicable).
  • 15.Demonstrates critical thinking and diagnostic reasoning skillsin clinical decision making by the identification, evaluation andmanagement of the individual needs of patients/families.
  • 16.Demonstrates the ability to utilize equipment in a safemanner.
  • 17.Bases decisions made/actions taken on the relevant scientificprinciples, established standards of care and/or researchfindings.
  • 18.Utilizes teaching/learning theory and appropriate teachingstrategies in the provision of education to nursing staff, hospitalstaff, students, patients, families and the community.
  • 19.Promotes an environment that facilitates learning by respondingto appropriate patient/family preferences, priorities and readinessto learn.
  • 20.Updates own knowledge base throughout the year.
  • 21.Completes all identified clinical competencies.
  • 22.Serves as a resource person for staff caring for thepatient.
  • 23.Provides educational programs to professional andparaprofessional personnel in area of expertise.
  • 24.Participates in community programs, distribution of literature,as appropriate in area of expertise.
  • 25.Documents the plan of care, interventions and evaluations in aclear and concise manner for every patient visit in the patient’smedical record following approved hospital/UHA policies andcompliance standards for documentation.
  • 26.Collaborates with other leaders and staff to effectively achievepatient outcome based improvements in care.
  • 27.Provides leadership in the development and implantation ofchanges in the telemedicine practice that positively impactspatient outcomes.
  • 28.Works collaboratively with medical and nursing staff tofacilitate creative problem solving and provide professionalsupport.
  • 29.Participates and initiates inter-disciplinary approaches topatient care, program development and education. Serves as aprofessional role model.
  • 30.Promotes and practices innovation of the expanded role in thedelivery of care to patients and families throughout the inpatienthospitalization, and the ambulatory setting, as applicable.
  • 31.Participates and provides leadership for committees and taskforces.
  • 32.Documents hours and submits billing reports, asappropriate.
  • 33.Reviews and interprets care data from sources such as patientregistries, clinical and financial reports, length of stay, accessto care, and cost per case reports and recommends/implementschanges as necessary.
  • 34.Participates in clinical trials and research studies with thePrincipal Investigator.
  • 35.Assists Faculty, Administrators and Directors in the collectionof outcome measures.
  • 36.Other duties/projects as assigned.
