Endeavor Health in Glenview, IL, is seeking an Advanced Practice Provider (APN/PA) for our Urology Research program to lead a precision medicine clinic using Polygenic Risk Scores (PRS) for high-risk patients. The APP will coordinate care, review PRS results, and guide risk-stratified testing and referrals.

Responsibilities include patient visits, ordering tests, data collection, and collaboration with Urology, Cardiology, and Gastroenterology, while ensuring regulatory compliance.

Qualifications

• License: APN or PA licensure in IL; Federal DEA and Illinois CS license required.
• Education: Master's level program in specialty area.
• Certification: BLS/CPR certification required.
• Experience: 0-3+ years of clinical experience in primary care, oncology, cardiology, or related specialty.

Responsibilities

• Conduct initial and follow-up patient visits to review PRS results and implications.
• Review medical and screening histories to identify gaps in preventive care.
• Order appropriate diagnostic tests based on PRS findings.
• Manage follow-up on test results and initiate guideline-directed therapy as indicated.
• Collaborate with Urology, Cardiology, and Gastroenterology to manage findings.
• Participate in team meetings and research data collection.
• Maintain regulatory compliance with SOPs and IRB requirements.
