Job summary

The Advanced Practice Nurse (APN) Certified Family Nurse Practitioner (FNP) works in Behavioral Health Service within the Veterans Health Administration to deliver a full range of services focused on quality and timeliness in service of the VA mission.

The role includes managing patient panels with acute and chronic conditions, developing care plans, ordering tests, prescribing interventions, and collaborating with interdisciplinary teams.

Responsibilities

• Manages a panel of patients with acute and chronic health conditions - focusing on health promotion and disease prevention using evidence-based practice.
• Develops and implements individualized care plans to achieve optimal health outcomes
• Orders and interprets labs - tests - and diagnostic studies
• Prescribes pharmacologic and non-pharmacologic interventions
• Initiates referrals and consults to specialty services as needed
• Assesses and adjusts care plans to ensure optimal patient outcomes
• Provides education to patients and their families regarding health conditions - treatments - and preventive care
• Demonstrates leadership and works collaboratively with interdisciplinary teams
• Leads and participates in performance improvement initiatives and quality improvement projects
