Responsibilities

Position Summary

The Anesthesiologist provides safe, efficient, high-quality anesthesia care to patients having outpatient orthopedic procedures at Capital Orthopaedic Surgery Center and District Surgery Center. Cases include arthroscopy, sports medicine, hand and upper extremity, foot and ankle, spine, and outpatient total joint replacement. This role puts a strong emphasis on regional anesthesia and multimodal pain management to support rapid recovery, same-day discharge, and an excellent patient experience. The Anesthesiologist works closely with surgeons, Certified Registered Nurse Anesthetists (CRNAs), nursing staff, and center leadership in a fast-paced, high-throughput setting.

Essential Duties and Responsibilities

Clinical Care

  • Perform thorough pre-anesthesia evaluations, including review of medical history, labs, and diagnostics, and determine each patient's suitability for the ambulatory setting under the center's patient selection criteria.
  • Develop and carry out individualized anesthesia plans, including general anesthesia, monitored anesthesia care (MAC), neuraxial anesthesia, and peripheral nerve blocks.
  • Perform ultrasound-guided regional anesthesia, including single-shot and continuous peripheral nerve catheters (e.g., interscalene, supraclavicular, adductor canal, popliteal, and similar blocks).
  • Use multimodal and opioid-sparing analgesia protocols, and support enhanced recovery after surgery (ERAS) pathways for total joint and other orthopedic cases.
  • Monitor patients continuously during procedures, and manage hemodynamic, airway, and other intraoperative events.
  • Oversee post operative recovery, determine discharge readiness using established criteria, and manage postoperative nausea, pain, and complications.
  • Respond to emergencies, including malignant hyperthermia, local anesthetic systemic toxicity (LAST), airway emergencies, and cardiachet events, and coordinate transfers to the center's designated acute care hospital under its transfer agreement.

CRNA Collaboration and Supervision

  • Serve as the collaborating anesthesiologist for CRNAs practicing at the center, consistent with Maryland Board of Nursing regulations
  • Provide medical direction to CRNAs as required by CMS Conditions for Coverage and center policy.
  • Supervise nurse anesthetist graduates (pre-certification), when applicable, with on-site and immediate availability as required by Maryland regulations.

Collaboration and Communication

  • Work with surgeons and perioperative staff to keep schedules on time, reduce delays, and maximize OR efficiency.
  • Communicate clearly with patients and families about anesthesia options, risks, and recovery expectations, and obtain informed consent.

Quality, Safety, and Compliance

  • Take part in the center's quality assurance and performance improvement program, peer review, and infection control activities.
  • Support readiness for Maryland Office of Health Care Quality (OHCQ) licensure and complaint surveys, CMS surveys, and accreditation surveys (AAAHC)
  • Maintain complete, accurate, and timely documentation in the electronic health record.
  • Follow all federal and Maryland requirements for controlled dangerous substance (CDS) handling, storage, documentation, and waste, and use the Maryland Prescription Drug Monitoring Program (PDMP) as required when prescribing.
  • Help develop and review anesthesia policies, protocols, and patient selection guidelines.

Administrative

  • Support accurate anesthesia coding and billing documentation.
  • Maintain medical staff credentialing and privileges at the center.

  • Two or more years of experience in an ambulatory surgery center or high-volume outpatient orthopedic practice
  • Experience with outpatient total joint replacement and ERAS protocols
  • Experience working in an anesthesia care team model with CRNAs

Knowledge, Skills, and Abilities

  • Strong clinical judgment in selecting patients for outpatient surgery and managing comorbidities such as obstructive sleep apnea, obesity, and cardiovascular disease
  • Efficient, organized workflow suited to a quick-turnover environment
  • Excellent communication and interpersonal skills with patients, surgeons, and staff
  • A commitment to patient safety, a positive patient experience, and a team-based culture
  • Comfort with electronic health records and anesthesia information systems, including use of CRISP, Maryland's health information exchange

Physical Requirements and Working Conditions

  • Ability to stand for long periods, and to bend, reach, and assist with patient positioning and transfers
  • Manual dexterity and visual acuity to perform invasive procedures and operate monitoring and ultrasound equipment
  • Possible exposure to bloodborne pathogens, anesthetic gases, radiation (fluoroscopy), and hazardous materials; appropriate PPE required
  • Typical schedule is weekday daytime hours with no overnight call; occasional early starts or late cases depending on the surgical schedule
