Job summary

WakeMed Health & Hospitals seeks a Wound Care RN to collaborate with physical therapists, physicians and advanced practice providers in the Raleigh OP Wound Care Center to manage patient plans of care. The ACN supports the PFCC model and coordinates care with families and the health care team.

The role emphasizes excellence in nursing practice, patient advocacy, and community-based care alignment within WakeMed's standards. EOE.

Qualifications

• Registered Nurse license required.
• 5 years nursing experience required.
• Wound care experience preferred.
• Hyperbaric Oxygen Chamber experience a plus.

Responsibilities

• Provide wound care in Raleigh OP Wound Care Center, collaborating with therapists, physicians and APNs.
• Coordinate patient care plans with the interdisciplinary team and communicate with patients and families.
• Support PFCC model and nursing practice standards within WakeMed.
