Job summary

Qualifications

• Completion of a Licensed Practical Nurse Program required or equivalent combination of education and experience.
• LPN – Licensed Practical Nurse - State Licensure in New York State (NYS) upon hire required.
• Successful completion of orientation at the unit level required.

Responsibilities

• In an outpatient setting, provides direct patient care under supervision of an RN or MD.
• Performs nursing interventions and participates in the plan of care.
• Documents collected data and medication administration per standards.
• Contributes to patient and family education and supports care delivery.
• Maintains safety, teamwork, and effective communication within the healthcare team.
